2025-26 Pre-Participation Information
Pre-Participation Physical Form | SportsWare Instructions
Welcome to Cal Maritime Athletics! Below is a checklist on how to get medically cleared for your sport. To receive medical clearance to compete in your sport, please follow the instructions below.
1. Physicals: All athletes need to complete an annual physical from a MD, DO, NO or PA to receive clearance. Please note that physicals completed by a chiropractor are not accepted by the department.

All returning students will need to complete a physical using the department's Pre-Participation Physical Form.

The Health Center is open during the summer and will conduct physicals on Mondays and Tuesdays for free. Athletes are highly encouraged to make an appointment by calling (707) 654-1170) since physicals are not offered by the Health Center once school starts.

Returning athletes who complete their physical outside of Cal Maritime will need to upload the paperwork into SportsWare or submit it to athletic training staff in person.

Make sure to retain a copy of your physical!
2. Pre-Participation Paperwork: All paperwork is done electronically on SportsWare, click here to access instructions.
- All returning athletes must log into their account, sign all of the forms and confirm that information is up to date at the start of every academic year to receive clearance.
- All new athletes will create a new account and follow the instructions provided, you can complete this requirement starting July 1st.
3. Concussion Baseline Testing: All incoming and new student-athletes must schedule and complete concussion baseline testing with the athletic training staff in-person during orientation. There is no need for returning student-athletes to repeat baseline testing unless a concussion took place at any point during the 2024-25 academic year.
