2025-26 Student-Athlete Pre-Participation Forms

Pre-Participation Physical Form  |  SportsWare Instructions

Student-athletes must complete the following requirements to become eligible for competition:

1. Physicals
Every student-athlete needs a physical conducted annually to receive clearance. Please note that incoming athletes will meet this requirement through their college entrance physical, everyone else must complete this requirement by clicking here and using this form. A MD, DO, NP or PA must perform this physical since Cal Maritime Athletics will not accept exams conducted by chiropractors.


2. Pre-Participation Paperwork
Student-athletes can submit pre-participation paperwork through SportsWare. New athletes must request access to this portal to complete requested paperwork. All returning athletes must log into their account, to update forms, in order to compete.

3. Concussion Baseline Testing
All incoming and new student-athletes must complete concussion baseline testing. The Department of Athletics is planning to conduct this during orientation.

There is no need for returning student-athletes to repeat baseline testing unless a concussion took place at any point during the 2024-25 academic year.  


Student-athletes who have any questions or concerns regarding physicals, pre-participation paperwork or concussion baseline testing should contact their head coach for additional information.
